Title Page: Title (in Bold), Author's Name, Institutional Affiliation,Course Number and Name, Instructor Name, Assignment Due Date, and Page Number in the Header

Rules

When citing directly, include: author, year of publication, and page number. 

When paraphrasing, include: author and year of publication. Page number is recommended by not required. 

With a signal phrase: According to Cummings (2000), "Standards tell us where we're going--what students should learn. They don't tell us how to get there" (p. 1).

Without a signal phrase: The author states, "Standards tell us where we're going--what students should learn. They don't tell us how to get there" (Cummings, 2000, p.1).
